Community Networking

Community Networking is a CARF accredited service focusing on enhancing an individual’s participation in community activities by building on their unique interests, strengths, and goals. The service aims to connect individuals with both formal and informal community supports, helping them develop the skills necessary for integration and sustained involvement. This involves facilitating access to local resources and supporting individuals in cultivating relationships that foster community inclusion.

Key outcomes of Community Networking include establishing positive relationships, engaging in meaningful community roles, and participating in preferred activities, organizations, or groups. These outcomes are achieved through a person-centered planning process that aligns with the individual’s goals, as outlined in their Individual Support Plan (ISP).

The goal of this service is to gradually reduce paid support and transition to natural community support when feasible. Individuals receiving Group Home or Independent Supported Living services are not eligible for Community Networking, as it is already a component of those programs.

Community Networking staff are required to be at least 18 years old with a high school diploma or equivalent. They receive specific training in areas like implementing Individual Support Plans, abuse and neglect prevention, CPR/First Aid, medication administration, and positive behavior support.
