Community Living Services

PCBS Community Living Services are CARF accredited and offer a wide range of personalized living support options, available either in the individual’s home or in properties owned and operated by PCBS. A team of qualified professionals collaborates closely with the individual and their support team to develop a customized plan that outlines the specific services to be provided, ensuring that each person’s unique needs and preferences are met with the highest level of care and support.

These services are designed to maximize opportunities and promote community integration. They may be delivered through the Group Living program, which provides individualized support and 24-hour supervision in agency-owned group homes. Alternatively, services are available through the Individualized Skill Development, Individualized Supported Living, or Personal Assistant programs, which offer support in the individual’s family home, personal home, or apartment, with services tailored to meet their specific needs. These programs provide a range of support options, from just a few hours of assistance per week to comprehensive 24/7 care.

The Community Living Services programs are licensed by the Missouri Department of Mental Health and are primarily funded through the Home and Community-Based Waiver. Additional funding is provided through the Senate Bill 40 tax levy. To receive full funding for residential services, referrals must be submitted through the Kansas City Regional Office, specifically via the Center for Human Services.

Service Description: Group Home

Group home services offer comprehensive care, supervision, and skills training in areas such as Activities of Daily Living (ADLs), home management, and community integration. These services include assistance and support in self-care, sensory and motor development, interpersonal skills, communication, community living skills, mobility, healthcare, socialization, money management, and household responsibilities.

Service Description: Individualized Skill Development

Individualized Skill Development (ISD) services provide tailored supports to individuals living in their own or family homes, helping them acquire, build, or maintain the complex skills necessary to maximize personal independence. Teaching methods are customized based on the individual’s goals, learning needs, and desired changes, as identified through the person-centered planning process. These services are delivered in alignment with the Individual Support Plan (ISP) to achieve the specified outcomes.

Service Description: Individualized Supported Living

Individualized Supported Living (ISL) services offer personalized supports to individuals living in homes of their choice. Those receiving ISL services have the autonomy to decide with whom and where they live, as well as the types of community activities they wish to participate in. This service provides the necessary assistance and support to help individuals achieve personal outcomes that enhance their ability to live independently and actively engage in their communities.

Service Description: Personal Assistant

Personal Assistant (PA) services offer a range of assistance for Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to help individual’s complete tasks they are unable to perform independently due to a disability. These services are provided in the individual’s own home, family home, and/or community, and must always be delivered in the presence of the individual.

Staff Qualification:

Community Living staff are required to be at least 18 years of age and possess a high school diploma or equivalent. They undergo specialized training in key areas such as the implementation of Individual Support Plans, prevention of abuse and neglect, CPR and First Aid, medication administration, and positive behavior support to ensure they provide the highest level of care and support to individuals in their care.
