Community Networking

Community Networking is a CARF‑accredited service that helps individuals take a more active role in their communities. It builds on each person’s interests, strengths, and goals. The service connects people with formal and informal supports and helps them develop skills for long‑term involvement. Staff also guide individuals to local resources and help them build relationships that support true community inclusion.
Community Networking encourages people to form positive relationships, take on meaningful roles, and join activities, organizations, and groups they enjoy. These outcomes grow from a person‑centered planning process aligned with each individual’s goals in their Individual Support Plan (ISP).
The service also works to reduce paid support over time and shift to natural community supports when possible. Individuals receiving Group Home or Independent Supported Living services are not eligible, since Community Networking is already part of those programs.
Staff must be at least 18 years old and have a high school diploma or equivalent. They complete training in ISP implementation, abuse and neglect prevention, CPR/First Aid, medication administration, and positive behavior support.
